We are pleased you are considering or have chosen Corban Counseling for your counseling needs. The decision to come for counseling is quite important and we are honored you are evaluating our practice. We want you to have all the information you need in order to make an informed decision. We will provide you all the necessary information and forms on our website for easy reference.
When you call or email for an appointment we will schedule you as soon as possible with your therapist. Sometimes there may be a delay before your first appointment. We recommend you schedule several appointments at a time to ensure you have the most convenient time for you and your family. There is often limited availability and it is frustrating once you begin to have further delays.
On our site you will find a list of our fees, forms to download, and insurance info. The fees listed are for self-pay clients but if you have insurance your fees will be based on your plan. With many plans you would pay the specialist co pay if you have met your deductible. We are currently accepting Blue Cross and Blue Shield, Aetna, and United Healthcare. Please bring your insurance card to your first appt. There are intake, disclosure and information forms available for you to download, fill out, sign and bring to your first appointment as well. If you plan to use your health insurance, please consult your plan information for your co pay and deductible.
You can pay with cash, check, Visa, MasterCard, Discover and Debit. We do not accept American Express. You have the option of using your HSA or FSA card as well. Thank you again and don't hesitate to give our office manager a call if you need help or have questions. 704-560-8706
